Look at the health bar, he's playing on Extreme. Johnny can't hear Snake's footsteps because he is using the D-pad to move around instead of the analog stick.

I don't think he was playing on the easiest difficulty because his health bar was quite low.  EDIT: -3lon beat me to it.

maybe its just because johnyy is a different setting to the rest of the soldiers on alert usually the enemy knows exactly where you are |

Thats true it probably was a different setting or Kojima did it on purpose so that someone could actually do that to him. |
