Something happens in the future that allows Big Boss and Miller to join Foxhound, probably at the same time.
possible answers:

-Perhaps they can't take the hunt anymore. They form a plan to take off the heat.
-Their plan is to pretend to shutdown MSF and rejoin the Patriots ranks, while MSF secretly becomes a new group, Outer Heaven.
-The Patriots welcome back BigBoss because he's a valuable icon despite his unwillingness to cooperate in the past. They recommend that Miller change his first name for security reasons. They don't want nosey people to think that the American government would simply enlist Kazuriha Miller with no questions asked.
-As the years go on, BigBoss, in the development of Outer Heaven, commits more and more acts that would make one (Miller) view him as "a monster". Kidnappings, etc.. all in preparation for the Uprising.
-Miller ends up refusing to work with BigBoss any longer with OuterHeaven. Perhaps BigBoss doesn't even want to run it as a business so much as a place where Soldier's will always have an excuse to fight.
-Perhaps Miller denies BigBoss's request to go to OuterHeaven during the uprising in 1995 and leave his safe life in the US behind.

Perhaps the reason they decided to show us the timeline of Miller's death was to show how Miller would eventually give up on BigBoss, and how he would be viewed as an expendable traitor by BigBoss's son.

Well, technically, Roy Campbell mentioned similar things to Solid Snake during that fight.

And anyways, we'll need more information before we can truly deduce what exactly happened. For all we know, Miller may have ended up being forced into it because the Patriots held his daughter hostage, similar to how they persuaded Campbell to participate in the Shadow Moses Incident (since the instruction manual mentioned that he had a daughter that lived with him in Los Angeles, which is a strange thing to note given the fact that she's not even referenced at all.).

I bet Big Boss did something to really pee' off Miller, maybe he frowned upon the Outer Heaven ideas beforehand. It's all still to be explained...
