No actually it doesn't have to be explained, there's nothing obligating them to do so.

I think there's a distinct possibility that you're out of your damn mind if you actually think this could happen considering the fact that Gray Fox isn't a Nicaraguan like Chico is, and Chico is too young to be him anyway since Fox was already in his late teens during Portable Ops.

There is nothing obligating them to do it besides the unexplained pink elephant of a hole in his chest.

Regarding the possibility of Chico ending up being Gray Fox, obviously it would involve some retconning of Gray Fox's backstory. It is more a hunch than anything based on the idea that at the moment Chico is the least trusted. Ground Zero is a turning point for many of the characters Kojima says so maybe TTP will see a change in Chico to become a more trusted leutenant. Also Big Boss always seems to be coming to his rescue just like he should have been coming to Gray Fox's rescue around this time. There are other things too from Peace Walker which could be hints towards this but there is no "evidence". Like I said it is more of a hunch than anything given that we haven't seen Gray Fox yet.

given that we haven't seen Gray Fox yet.
Except we have, Null from Portable Ops IS Gray Fox, Portable Ops takes place in 1970 to 71, Null was at least in his late teens or early twenties which would put him at his mid to late twenties or early thirties in Phantom Pain, Chico is thirteen in Ground Zeroes, he's twenty two in Phantom Pain (that is if he lives) he's too young to be Gray Fox and their nationalities don't match up in the least.

And no, they don't have to explain the hole in his chest if they don't see fit to do so, the last time people demanded that they explain every little thing, we got the pile of shit that was MGS4's plot.

You do realize they haven't said anything at all about it right? What would be the purpose of including that if they were never going to even comment on it. That wouldn't make any sense. In MGS4 they tried to provide answers for questions that at least had been brought up in game.

Yeah...questions that were never meant to have real answers, which is why the answers of about 90% of everything was "Nanomachines."

The questions asked in MGS1, 2 and 3 all had multiple different clues that hint toward multiple different answers and the players were meant to figure out what answer made the most sense for themselves, MGS4 was made for that special group of people who demanded answers because they have an almost mentally handicapped need to have everything spoon fed to them because they can't sit down for two minutes and think something over for themselves.

Now consider the fact that Kojima is making this game how he wants it made, not letting dipshit "fans" decide what he does and doesn't get put in the game, which is a fact backed up by his ignoring all those petitions to get David Hayter back and ignoring all the complaints against things like reflex mode, enemy marking and regenerating health.

So no, he doesn't have to explain the hole in Chico's chest just because it was shown and people want an explanation for it, besides, it really doesn't need an explanation anyway, it was either self inflicted or done to him by Skullface or one of his cronies during one of their daily torture sessions and he sticks his headphones in there to keep them from getting ruined by the mud and all the other shit on the ground because his prisoner jumpsuit has no pockets. See? A perfectly logical explanation backed up by actual facts in the game and I didn't need to have Kojima spoon feed it to me.

He'd be a strong supporter in Kaz and Big Boss' quest for revenge.
You too have known loss, and that loss torments you still. You hope hatred might someday replace the pain, but it never goes away.

In Tape 6 we hear a drill and Chico shrieking in pain. I'm pretty sure the sound was not only the screws in his tendons, but also them drilling a hole in his chest. I'm pretty sure Chico keeps his headphone in there so as for them not to ruin and more importantly as self-punishment for having betrayed MSF and his regrets over taking Skull Face's offer to end Paz's suffering.

After everything Chico went through in Ground Zeroes? The kid was torture beyond words in the most inhumane ways, saw Mother Base destroyed, and believes its all his fault. And above all else, he saw Paz, the girl he loved more than anyone, be tortured and killed. That last one is most probably going to be the driving force of Chico's character in TPP. He will most likely hellbent on revenge against Skull Face for what he did to him and Paz.
