Ishmael is voiced by Sutherland.
Snake is voiced by Sutherland.

As Snake is waking up from his coma he's still in 'half' a dream state. That's why he sees Ishmael, Volgen, Giant Whales, etc. He's trying to escape and tripping balls at the same time. To me that's as clear as day.

Also, it's common to have hallucinations after a coma--

edit: WOW I just spent too much time in a coma forum reading about peoples horrible experiences… A lot have very vivid nightmares- some involving the hospital or something similar while in a coma. Even people fighting in the hospital came up quite a bit. Very scary stuff. After reading all this I'm not sure XOF attacks Snake in the hospital at all… The first chapter could all be 'coma dream'….
Yea, I'm 90% sure it's a hallucination and BB is just trippin tf out...but there is some form of invasion/escape because Ocelot has BB shooting his way out.
As far as "What about him?" I think it's Chico or the Medic, but let's think about it, Kaz isn't there when BB wakes up, so I really doubt that person will also be there unless they had an injury that kept them there for 9 years too..which is highly unlikely.
If anything though I just pray it isn't a double.

If BB is the one tripping, why can Ishmael and Ocelot see the flaming demon?

Snake, history will never know what she did. No one will ever learn the truth. Her story, her debriefing... will endure only in your heart. Everything she did, she did for her country. She sacrificed her life and her honor for her native land. She was a real hero. She was a true patriot. - EVA

If it were all inside of Big Boss' head, Ishamel and Ocelot seeing the Flaming Demon that Big Boss conjures wouldn't be far-fetched because they would be figments of his imagination too.

WHAT, SWEEP?!
Hey hey, everyone! This is something I've wanted to discuss here on the MGSForums for quiet some time now, but because I didn't like the idea of creating a thread with an insane wall of text that people might not even get a quarter of the way through before getting bored, I decided to make a video where everything fits together and flows much better. Namely, a video analysis of as much of the evidence in support of Ishmael being a hallucination as possible.

I am not claiming to have been the one to created this theory - I'm very well aware it's been around for a long time now - but, rather, this is my own take on the theory, analysing everything I could find from the three videos we have of Ishmael to support this.

ENJOY!

I really hope this isn't seen as advertising, or anything like that, but this is the best way in which I'm able to express myself without presenting a boring wall of text, so hopefully everything will be okay, and I would love to hear your thoughts and get a discussion going :D.

xx

Don't fight it.
I didn't read it all I'm afraid, but there were some interesting points being made. I'm generally leaning toward Ishmael being the Medic but the fact that Ishmael's voice hasn't been pitch shifted lower like the Medic's has, could mean that might not be the case.

Got it Memeorized?
At the end of the VGA trailer, when we see Big Boss being identified by the soldiers and Psycho Mantis-lookalike rising up nearby, the soldier aiming the gun definitely reacts and turns his face towards that direction before the screen cuts to the black. Obviously, we can't tell for sure because it cuts right after that, but if the soldier was indeed reacting to Mantis then he's real, not just in Big Boss' mind.

If we assume that Mantis is indeed real, then Ishmael not reacting surprised when he saw him near the elevator could be explained by him actually knowing about him, thus him not being surprised by seeing him. Also, if Ishmael is indeed real, then him not being intimedated by seeing the patient shot in the head while he and Big Boss were under the bed could be a result of him being experienced with that sort of thing. If you're an experienced soldier, then there is no reason to be surprised by seeing the inevitable. Also, if Ishmael was burning near the elevator as a result of Big Boss' mental state and shock, then he should also be fazed by seeing that soldier shot, yet he isn't.

Also, Mantis (if he's real) is most probably not with XOF. We see him on three main occasions. The first is when the elevator exploded, the second is when Snake was identified by the soldiers, and the third is when the chopper spots Big Boss. In all of these occasions, Big Boss is saved. The first time from the elevator explosion, the second from the XOF soldiers who identified him, and the third from the chopper which spotted him. The fourth time is on the horse chasing Big Boss. If Mantis (and demon Volgin) are indeed chasing Big Boss, yet Mantis saved him on a number of occasions, then the only explanation would be that Mantis (and whoever is with him) want Big Boss returned or taken somewhere. Maybe it's Cipher, which would be the most probable scenario.

That said, great stuff there.

Meh at this point i couldn't care less about any new theory. No offense, i'm sure you put a lot of work into the video.

Hideo never intended for us to be able to tell. There are things that point towards him being a hallucination, but there's just enough to make you question it. I wouldn't be surprised if we only got to find out who/what he is near the end of TPP.
PEACE will come.
