I was just watching one of the older trailers, I believe it was the one with Garbage, and when I saw the words "Venom Awakens" pop up it made me think that I've been over-thinking this whole "V has come to" thing all along. I really think it's just another way of saying Venom Awakens in regards to Big Boss turning evil. Most of you probably already figured this, but I didn't really start watching the trailers or anything closely at all until just the past few days so this almost comes as a revelation to me right now.
It probably just means Big Boss (Venom) has awoken from his coma, since IIRC it was in the first TPP trailer. I always imagined it was that nurse saying it too, but it's probably not.
The only reason i think it's one of the nurses is because the doctor has an accent on him. The female voice also seems to have one from what i can tell.

Kojima said V stood for Victory. Which I heard was something snake was called at some point in Peace Walker, in a shorter version i.e. Vic Boss. Now it's apparently Venom.

Yeah, it's probably as simple as that.
I definitely don't think it's as big of a deal as it was first made out to be.

Yeah, it's probably as simple as that.

I don't think the incision has anything to do with the phrase "V has come to."
But that's just me. I think the letter V will represent several things throughout the game (Venom, Victory, etc.), but I just don't think the phrase "V has come to" is as significant as it seemed originally.
Only thing i can think of is one of the nurses is an agent for cypher and when Snake awakens it's the message she sends to Zero, "V has come to" meaning (Venom) Snake regained his consciousness. Mark my words Zero has a hand in keeping Snake alive and well during the coma. I'd be shocked to find out otherwise.
That's my theory.
And how does said nurse know that Big Boss' new code name is "Venom"?
3. Third strap on eye patch, and horn. Did Big Boss just add an arbitrary third strap and later revert to two again and have the horn removed?
4. During a soldier interrogation in a TPP gameplay video, Venom Snake must translate what a Russian soldier is saying. Big Boss knows Russian. Some people have speculated this is due to amnesia from the chopper crash. If true, is it things from the past including his linguistic skills or just the events happening during 9 years in the coma?
5. Big Boss doesn't look like he has a prosthetic arm, a 3-strap eyepatch or all Venom Snake's scars during MG 1 and 2 and at the end of MGS4.
I've pretty much been on the side of it meaning Venom has awaken. As in, this is when Snake turns into Big Boss and the Venom persona is that bridge into the the Big Boss era.
